Basic Information:
• Name, breed, age.
• Gender (neutered/spayed or not).
• Does the pet have any special habits or personality traits?
Feeding:
• How often and what should the pet be fed?
• Does it have any allergies or forbidden foods?
• Does it have a special feeding schedule (meal times, portion sizes, etc.)?
Health and Care:
• Does the pet have any chronic illnesses or require medication?
• Does it have any environmental allergies (dust, grass, etc.)?
• Is coat care needed (brushing, bathing, etc.)?
• Emergency contact for a veterinarian.
Walks and Activity:
• How many times a day does the pet need to be walked?
